- A child is defined as under 14 OR the age at which the child still has baby teeth present. Adult dental age is defined as 14 or older OR when all permanent teeth are present- whichever is first. If a child becomes an adult dentally DURING their membership year, benefits will convert automatically to that of an adult plan at NO ADDITIONAL CHARGE. At the next renewal, the child will be converted to an adult plan.

- Dysport can be administered no sooner than every three months
- Units are non refundable
- Units are non transferable
- Units cannot be redeemed for cash value or other esthetic services
